You should show your GEHA Connection Dental FederalSM member identification card each time you or a dependent sees a dental provider to make sure that you receive the appropriate benefits. For your convenience, you can print out a temporary paper ID card if you:
Lost or forgot your member ID card.
Need an extra copy to send with a dependent.
Need dental care and you're away from home without your ID card.
Print-outs of GEHA Connection Dental Federal member ID cards will show only the plan member's name, but may be used by eligible dependents.
Temporary ID cards printed from this site will expire in 30 days. To get a replacement ID card by mail, please call (877) GEHA-DEN.
